A 2 week notice of resignation is appropriate. I do not think an explanation is required, but If it makes you feel better you could tell them that you need to spend sometime with your daughter while exploring other possibilities. They are not entitled to anything more than that. Hang in there, and remember that " life happens " with or without surgery. There is also a grant available for professional people to continue their education..... for NH residents (maybe Mass might have it too... I don't know). My sister had two degrees and was able to get this grant for $6250.00 to go for more education - that's how I know about (but she moved before she could use it). That is how much they allow per person. It is called H-1B Technical Skills Training Grant. if anyone wants to check into it. I don't know how she found out about it, but she e mailed me the applications so if anyone from NH is interested I can always scan and e mail them so you can print them out like I did.
