Make sure it is coded correctly - there is a file i nthe insurance section (1st steps ...) that has the diagnosis and treatment codes. Ok call your husbands benefits coordinator (he can ask HR for the phone number). Have them look up the info for you. You have a right to it. We had this same issue - an exclusion. Turns out the insurance company had an old contract from my company. It was supposed to be included at a $20 copay. So it's worth the hassle. It took 4 months to determine it was the wrong contract and then another 4 months before all the paperwork was straight and we got all our money back.
