Good information and advice! My surgeon sent in a ton of stuff the

day before I sent in my appeal letter (faxed and mailed). I will

check into the taping thing (I don't think it is legal in this

state, but I will double check) and will try to get ahold of an

e-mail address for them to get everything in writing.
