RE: confidentiality contract

I asked the other day if anyone had a copy of a confidentiality contract and

a wonderful person from the list sent me a copy of one (thanks ) and

said it was okay to post it to the list ... so here it is! I just put my

name in and today's date, but you get the idea.

CONFIDENTIALITY AGREEMENT

Confidentiality is a quality of private information that is derived from the

patient during discussions with the hospital or clinic representative with

the explicit understanding that this information will not be disclosed

except for the purposes (such as receiving healthcare services) for which

said information was provided.

It is understood and agreed that BRANDI HOUSER has on this date, January 14,

2003, agreed to complete dictation tapes from (hospital or clinic name)

..

It is further agreed that the patient’s rights to confidentiality and

privacy are to be protected at all times throughout the preparation

(dictation and transcription) and storage of the documentation that is

provided to BRANDI HOUSER in any cross-training, full-time, part-time, or

prn employment and that BRANDI HOUSER will maintain complete confidentiality

of all medical records, transcripts, dictation and any other information

received in association with completion of this assignment.
