Now I am 46 - the top of the femur looks more like a mushroom than a ball and there is also damage in the " cup " area. Dr. Mont (who I realize is very conservative) called it 50:50. I think maybe I've had the pain so long I've grown used to it. I can still walk but the distances are diminishing quickly. 8 or 9 years ago I thought nothing of a 10 mile hike. Now it takes most of the day to recover from a 6 block walk from the subway station to my office. I've had to give up pretty much everything else. Mostly I have a dull aching pain in my hip - especially bad at night. Occasional sharp pains when I catch a " rough spot " . Frequently the pain is worse in my knee and it often extends all the way down to my ankle. He told me no, flat out. My hip was too messed up, deformed, and degenerated...I had almost no head left, and because of my problem, I had very little neck. So ironically, the THR restored more normal mechanics to me than a surf would have. De Smet explained that to me quite carefully. So there are some cases where a surf is not automatically better than a thr. But if a surf is possible, I always think go for it! I have large bearing because I'm flexible, and m/m because of my age, 36. Yep, pretty young. My Biomet is similar to the model, I think. I have a surf style cup, and the head is 38 mm, which is what I would have gotten if I could have gotten surfed. During my initial visit my OS (Dr. Helmy) thought I would be a candidate but was not sure due to the mushroom type deformity my femoral head had become after limping for 30+ years from a misdiagnosed SCFE, which went untreated for 9 months. Dr. helmy was going to speak at a conference sponsoner by Corin in the UK and he wanted to present my data, x-rays, 3-d cat scan etc as a case study and see what the OS's with more experience with resurfacing felt. When he returned, he called me and told me I would be a candidate. I still understood going into it though if he opened me up and found something unexpected, that I may still receive a THR. Well after the surgery, I woke up to a new Corin C2K resurfaced hip. Things were progressing nicely until 4 months post op, out of the blue, I fractured the femoral neck and had to be converted to a MoM THR. The cup side was ok, so I just received a stem and new ball to match up with the already in place cup (48mm). Not everyone is a candidate for resurfacing, regardless of where they go and who they have do it. There are many highly skilled OS out there but none of them can fix(resurface) everyone. After many discussions with my Dr. it was decided that I was probably on the extreme end of being a good candidate. I have no regrets in the decision I made and would do it again if I had to do it over.
