What is causing you to be hyper DOES make a difference, treatment is different depending on the cause... If you are hyper cuz of Hashi's, using RAI to kill the gland, in my PERSONAL opinion is criminal. It's not that much better if it's Grave's... if there is NO other way, if you are in thyroid storm and can't have surgery..... if you are dealing with cancer, the surgery should be done first and then a LOWer dose of RAI can be used after to kill off remaining tissue..... But... my opinion here... to say RAI is the only option, that surgery is out, and that it doesn't matter what is causing the gland to over produce AND top that off with not understanding how to use tapazole..... My eyes see a doc that just wants to prescribe pills, the least effort to her.... I agree with Topper though..you can not treat hyper properly with out finding out the reason why first.. Being hyper is just a symptom..your thyroid is producing too much thyroid hormone.. Grave's Hashimoto's Genetics TSH producing pituarynoma. Ovarian Strumillis ( an ovarian cyst with thyroid makeup that actually produces a form of TSH stimulating the thyroid gland) A toxic thyroid nodule. Cancer( rare in hyper but possible) Each one of these has a different treatment plan..RAI will destroy the thyroid and solve the symptom of hyper but it will not solve the problem..and although RAI is a viable option under certain conditions it is not a treatment plan. Grave's..ATD, surgery or RAI..ATD's should be the first line of defense..RAi is usually reserved for those who fail to respond to ATD's or are unable to take ATD's and for who Surgery is not an option. Hashimoto's..Synthroid or other thyroid replacement hormones. The idea even if hyper is to give high enough dose to supress the gland so the antibody's do not stimulate the gland..TSH stimulates the gland so the TSH needs to be kept suppressed under 1.0. Genetics..Surgical removal of the gland or RAI when surgery is not possible..ATD's can be used but would depend on how the person resonds to ATD's. Removal of the TSH secreting noma will usually result in normal thyroid function/ same goes for Ovarian Strumillis. Toxic nodule..this really depends on the size, location and how many nodules there is..it is possible to remove a single nodule and have a patient return to normal thyroid function.. Cancer..surgical removal of the thyroid followed up by RAI to destroy any thyroid tissue remaining in the body. Full replacement dose of thyroid hormones to keep the TSH suppressed and under 1.0. It's good measure to have a good diet, but with dangerously high blood pressure, it needs to come down as of right now, not a month or two or six months from now. Why would you not be a good candidate for surgery, has she told you? What are your thyroid lab values on the Free values of the thyroid hormones, or did I miss this? Never accept a verbal rundown of your lab values as "high", "normal", or "low, look at them for yourself. The TSH is not the indicator for this, especially if the pituitary itself is sick, and it will get sick with chronic hypo. It IS important for you to know the cause, and, if she's going by the TSH only, then who knows if you're even hyper or not. I would find another doctor, since she told you it wasn't important to find this out. This shows that she's not respecting you as a patient, if she thinks you don't need to know these things.
