Dear Sagliettigirl:

Most of us have been there. The advice I will give you is go get your hip

resurfaced in Belgium regardless of what a California OS tells you and then

come back and laugh in his face.

If I were you, I'd never go back to a doc for aftercare when you've already

gone against his advice to get resurfaced.

Sounds like if you want sympathy from that kind of doc, you'd be told to look

in the dictionary between shit and syphilis. Excuse the vulgarity.

I also suggest you shop around for a sympathetic OS so you can get whatever

aftercare you might need (probably none). There are some enlightened ones out

there. If you're in the Bay Area, I can recommend one highly. If you're not,

I can probably ask him for a referral. Let me know.

Best of luck,

I'm really sorry your original OS took that standpoint. Maybe you could

contact a resurf. manufacture rep. and get a referral. I will add that my

original OS (Dr. Stocks in Houston) was 100% for my upcoming resurf.

even though he doesn't do the procedure. He fully supported me, and the

procedure, agreeing to tend to my after care and going further to say if I

needed anything prior to surgery, to please give him a call. I think it's

interesting to note Dr. Stocks specializes in hip surgeries for young adults.

You can do a google search on him to read his bio.

The interesting thing I point out is that even though he doesn't do resurf.,

it certainly appears that he is fully supportive of the new procedure. When

I decided on resurf., I was concerned that I wouldn't have Dr. Stocks as my

doctor anymore. He's a fabulous guy and I'm glad I found him. However, I

have a sneaking suspicion that Dr. Stocks is in the process of training for

the procedure. Truly just a guess though.

I know this doesn't help you much in your quest for your follow-up care, but

I just wanted to let you know that there are surgeons out there that will

help and I certainly hope you can find one.

Take care,

I just joined surfacehippys about a month ago. I am from CA and my OS is the

one that picked this surgery for me. He said that it would be the best thing

for me because I was so young. I have read so many emails about people

traveling for this surgery. I had no idea that this was such a hard surgery

to get an OS to do.

Your OS is totally uninformed. The failure rate with BHR is very

small ... I don't have the stats to hand but it was <0.1%. Secondly,

some insurance companies won't pay for the procedure because it's

still in 'tests' in the US and is classed as 'experimental'.

Finally it's unlikely that you will need much in the way of

aftercare. I saw Mr Treacy at 7 weeks post op and then at 1year... my

next appointment will be at my hip's 3rd birthday. Mr Mcminn sees

his patients even less frequently. I'm sure that you will find a

sympathetic OS to look at your xrays at 6 weeks post op and 'giggle'

your hip about a bit!!!

Good luck and don't be put off by someone who is totally uninformed.

this is very common in OS that dont keep up-to-date - just find for

yourself the failure rates for resurface and for THR for people your age

and your (required) level of activity.

YOur OS may be looking at the failure rate for hemi-resurfacing and/or for

the 'old' resurfacing - pre McMinn.

confused again

I just returned from the appt with my original OS ..who in short told

me I was insane for considering to travel to Belgium to get an

operation that has a failure rate so bad that that is why the

insurance companies wont cover the claims .and blah blah blah..he

totally tried to disillusion me and was very un-supportive..Im really

upset but still feel that I should share so that I can

hear ..comments..Anyone out there been there? my motive was to rely

on him for my aftercare..Im feeling really confused about this..

He said alot of terrrible stuff to Promote THR and imply that BHR is

a silly idea...Ok so there must be some of you that has come up

against this resistance?Or is it only in CA.

Im scheduled for BHR in Belgium on the 11th of March...

Hi,

Shop around, I am 31 and have seen several OS and two of them

suggested really crazy osteotomies with little chance of success. One

of them actually proposed a series of three or more osteotomies over

2-3 years...CHOP IT UP...LET IT HEAL, CHOP IT UP AGAIN AND LET IT

HEAL AGAIN, LETS CHOP IT UP ONE MORE TIME AND HOPE IT HEALS, NOW WE

MIGHT NEED TO CHOP IT AGAIN, OOPPPS IT DIDN'T WORK, LETS DO A THR.

Their attitude was that if rearranging the hip joint by chopping and

pasting the bone didn't work, I could always get a THR..... they were

completely ignorant of the existance of resurfacing and dismissed it

as " artificial and therefore unreliable "

Any idiot will understand that the chances of a successful THR after

resurfacing is much higher and probably faster than it would be after

letting a whacko repeatedly play with making a jigsaw puzzle out of

your hip bone and then trying to stick it back together again... or

am I mistaken? I think this proves that there are many OS who are

looking in the wrong direction, don't take what he told you

seriously, just accept it as a sure sign that he is not going to be

of any help to you and look elsewhere.

DeSmet has pubblished the results of his recent resurfacings and they

are very very convincing.

Dear Saglietti girl,

I'm afraid your original OS is misinformed, quite badly so, and giving you the

forecful 'benefit' of information that is incomplete and wrong. The failure rate

of BHR is pretty low, in fact, remarkably low. Dr De Smet has performed over 600

of them and his failure rate is in the low single digits. I met consultants in

the UK who dismissed BHR as experimental, untried, risky, etc. It was

ill-informed and incorrect comment. It took some courage to reject their advice

and beliefs and continue with the BHR. It was, though, the right thing to do.

Here I am, 12 months later, rock climbing and mountaineering. Twelve months ago

I could do neither and was facing, if I had chosen the hip-replacement route, a

ban on doing them. The THR advocates were wrong. It was as simple as that. Take

heart and get you gone to Belgium.

With best wishes,

Chris.

I found one who fits that description. He told me honestly that he wouldn't

do the operation himself but was extremely supportive and joined my cheering

section before I left for my BHR in 2001. It was refreshing after hearing

from others how unpredictable and unproven the BHR procedure was.

In fact my own general practitioner initially refused to give me the referral

slip needed to see a specialist OS in Britain and when she finally did, wrote

in ALL CAPS " At insistence of patient " .

Your surgeon is talking out of his proverbial rear end. The failure

rate is certainly no worse than for THR's and in your profession

shouldn't you be looking for a procedure where the surgeon encourages

you to exercise hard afterwards? Not so with a THR. I have heard

McMinn described as a 'charlatan', but for me, and thousands of others,

he has been our 'saviour'.

I was told by my original OS that the BHR was unproven and not worth the

risk. Well, I listend to McMinn and others and decided the risk was

worth it. Absolutely no regrets whatsoever, 3.5 years down the line.

I am extremely active, but with a THR would be frightened to put my body

through the activities I undertake now.

In the UK, the operation went through clinical trials and is now

becoming mainstream for younger patients. Doesn't this say something?

Don't forget that every surgeon has a vested interest in his own

procedure. You're basically saying " Look, I don't want to follow your

advice, but when I've had this new type of operation, will you take care

of me? " . A slight to his professional pride... And his pocket, I

suspect! Fortunately, there are more enlighteded surgeons around.

But most importantly, you must be confident that you are making the

right decision. Weigh up all the options and risks yourself. The list

can only give you advice based on people's personal expereriences and

knowledge, although there's a huge amount out there to be gleaned. Much

of it is very well researched and I believe invaluable, but we are not

OS's.. At the end of the day, you must take the decision based on the

research you've undertaken, which may include communicating with other

surface hippies. Nobody else can do this for you. And I hope you will

then go to Belgium feeling good about your operation and the quality of

life you can then look forward to.

And even it fails some way down the line, are you any worse off than if

you'd have had a THR? I don't think so. At least this way you keep most

of your femoral head intact, rather than the comparitively barbaric

lopping off of it, and a metal spike driven into your femur. (No

disrespect to THR's meant, but by comparison, the operation is more

invasive and many of us are lucky to have the alternative choice)

Good luck! From reading of other's experiences, Belgium sounds like a

great place to be resurfaced.

S--The resurfacing they are putting down is the only resurfacing they

are familiar with. The Wagner and the Townley device were two peas in

a pod. The failure rate was about 60% at 5-6 years due to osteolysis.

That's pretty bad numbers but poyethylene in the 70's was to blame.

Of the surgeons I talked with, two of which had installed the Townley

from Depuy, they were both disillusioned with the idea. At the point

that I spoke with each of them, I failed to distinguish between that

and " metal on metal " resurfacing.

I have since spoken with one (did some roof repairs on his building).

He was interested in mom resurf, said it sounded like a viable

alternative.

I don't think the " failure rate so bad " belongs to our resurf. You're

on the right track. I think I would still try some other offices

beside JRI before I went to Belgium. It sure would be nice to have

insurance cover it. I haven't got unlimited funds. This bad hip

nearly made me a dependent not the breadwinner.

And it doesn't seem that JRI gives a rip whether they are paid cash

or insurance money. Heck lots of times insurance co's negotiate a

lesser rate for the same procedure. Given their track record on

getting (not getting) coverage, it would be wise to shop around, you

might be surprised.
