When had his first one a yr ago, it was a real eye-opener, turns out we had miscalculated his cal need by about 300. He was 8 in November, is around 53 lbs (about 35-50%) for weight, and about 10-15% for height. That's why we've kept them where they are, but did you actually adjust by 300? How is anyway? Guest guest Posted January 24, 2004 Report Share Posted January 24, 2004 Ree dx measures resting energy expenditure, ie - gives an individual metabolism rate, then percentages are added on for activities, (certain amount for sleeping, certain amount for age related activities etc etc) then 25% deducted (for keto diet restrictions) off the total, and you are left with an approx appropriate cal level. Painless, non invasive, you know - the ones we don't mind our kiddies having They get this helmet thing put on them, (like an astronaut's, so works well with the boys)...and the co2 expelled is measured. The child has to be kept as still as possible, (done for approx 20 mins- half an hr in our hosp) and when it is done properly, quite a few restrictions are placed on activities they can do beforehand. Like at CHOP, where the ree dx is apparently used quite frequently to assess cal level, (upon initiation and then when fine-tuning) I think the child has to have it as soon as they wake, not even allowed to walk to the bathroom beforehand etc. Our machine here in Auckland isn't that flash, and not quite the same attention is paid to detail, but we think it is still a lot more reliable than working on a table and having to guestimate. Meds seem to play a big part in the metabolism, and therefore the cal requirement rate, as does the amount of muscle tissue the child has - ie - the less muscle mass, usually the slower the metabolism. I remember a Hopkins paper from about 6 mths ago on the diet mentioned it as a possible tool now for setting calories, in that study they found that the previous cal levels (based on RDA tables and age) set for quite a few kids were shown to be a fair bit out of the ballpark. We did end up adjusting by the full 300, approx 50 cals every week or 2, but during the last nightmare hosp admission, because he was so inactive in bed, 's calorie level was cut way down (actually - the ree dx was also used while he was in the coma - this is the main purpose of it at our hosp, to test cal requirements for v ill folk to make sure they get sufficient cals to keep them nourished) and we are now in rebuild mode to get the cals back up again. He is due another test in a few weeks once we feel that the muscle mass he lost in hospital has been regained, and also to see what has happened metabolism wise after ditching the sluggish inducing benzos and replacing with the speedier metabolism inducing topomax.... He is doing ok, almost back to his old self, in some ways a lot better than he was before the status. I posted some pics in the photos section a few days ago - looking bright eyed and bushy tailed in the most recent one Seizure free as far as I know, but no clue what is going on underneath - an EEG is due next week to see how he is doing sub clinically.
