Re: the TV - we use captioning and have for a number of years. Both boys are extremely good readers so I think it's good to have the captions on early on so your child can associate words with what's being spoken. I don't know if she/he will read earlier or better but it can't hurt! Both our boys have telecoils programmed in their hearing aids (Tom has Phonak Claro 311 aids; Sam has Phonak Supero 412 aids). They use the telecoil in several ways - for the phone (we've found a 900 mhz or greater cordless phone works pretty well). I recently discovered - duh! - that our 900 mhz phone has a headphone jack in it so after three different adapters from Radio Shack, found a mono-to mono adapter that works with their telecoil neckloops. They set their aids to telecoil and then put the neckloop on so it makes phone conversations doable where before it wasn't very easy.
