hysterosalpingogram questions (M/C, child, preg mentioned)

My daughter had surgery to remove a septum about 7 years ago due to

4 M/Cs (three first trimester and one second trimester). The dr. did

not do any testing after her resection to make sure that the septum

was gone. She became pregnant a few months after her resection and

delivered a son 7 weeks early due to placenta abruption. She began

trying to get pregnant again about 4 yrs ago. She was able to get

pregnant twice, but both ended in M/C (7 weeks/9 weeks). During an

ultrasound with the last pregnancy loss, the technician described

her uterus as bicornate. Her dr. said that she thinks there is

still a septum there and this may be what caused her problems in

her pregnancy with her son. If not septum - possibly scar tissue.

My daughter is going for a hysterosalpingogram tomorrow.

Questions:

1) I am worried that her dr. will not give her an antibiotic after

her test. Is this routine? I am worried about scar tissue. Her

dr. refused to give her an antibiotic with her last D & C.

2) How good is this test for showing scar tissue or adhesions?

3) Is it ok to be on the birth control pill when having this test?

4) She is having this test on day 11 of her cycle - not the 8 - 10

day recommendation. Is this ok?

-> The doctor should prescribe antibiotics. I don't know for sure, but I do

think this is a routine preventive measure. I took antibiotics after my HSG,

but not after the D & C, if that helps any.

-> I don't know.

3) Is it ok to be on the birth control pill when having this test?

-> I don't know, but the place where she is having the test done should be able

to answer this question.

4) She is having this test on day 11 of her cycle - not the 8 - 10

day recommendation. Is this ok?

-> As long as she is not pregnant this should be okay.

-> In case her Dr. didn't recommend it (and mine didn't), she should take

Ibuprofin 30 minutes before the test to help with cramping.

I did not have an antibiotic after my HSG, my doctor never mentioned

a need for it. But hopefully more people will chime in. I don't

know how well scar tissue and adhesions show up on an HSG, but I do

know that they told me I had no polyps, so if they can tell you

don't have them then I would imagine they could tell if you did. I

would imagine it is ok to be on birth control pill when having the

test, as my OB wanted me to go on the pill while I was getting all

my tests done to ensure I would not get pregnant. I chose not to go

on the pill, but I would assume it is ok since my OB had suggested

it. If she is having the test done on day 11, that should be fine

as long as she knows she is not pregnant, that is the only reason

they like to do it between days 8-10. I agree with Mikell that

taking ibuprofen before the test is a good idea. I took it

immediately before and I think it was too late. I had severe

cramping during the test (but the test only takes like five minutes)

and I was fine after. Hope this helps some,

My doctor prescribed antibiotics to start the day before

the HSG as a precautionary measure. But, my doctor was

also aware that I am very prone to getting infections and

was fearful of getting one. Have her ask her doctor for

antibiotics as a precaution.

Your worry is founded in some fact, although infections following HSG

are very uncommon. I had 3 and my OB-GYN did not offer an

antibiotic; however, I asked for and received some each time.

Very good for showing intrauterine contours, although some very filmy

adhesions can be missed or incompletely revealed. Some flat scar

tissue may not show up.

Should be all right. I think I had one on CD 11 and things looked

good. Especially if she is on birth control pills, her lining should

be a bit thinner than normal, and good for viewing irregularities.

I'm sorry that your daughter has had such tough luck with her

pregnancies. PErhaps a more thorough workup would be in order--to

find out if she might not have some other conditions, such as auto-

immune or blood clotting disorders, that could also cause repeated

miscarriages.

It is possible for septum remnants to work against a pregnancy, but

most women who have large septa resected retain the total amount of

tissue in their uteri--it just is retracted into the walls and

covered over with endometrium.

Probably by now you know how the HSG went. Hope all is well. Good
