Guest guest
Hi Ram, I am afraid that Pierre is our resident BP expert, and he is in the midst of moving so he won't be on line for a couple of days. I don't know if Bonnie can help out here or not. I can tell you that for me, I have had to mix and match several different drugs before I found a combination that worked to get my BP below 140/90. I take Cozaar, Lopressor and Plendil, and that seems to have done the trick. I have also tried adding a diuretic in the past. I think the doctor has to play with the meds and see how your body responds. Sorry to not be of more help, but this is really an area of weakness as far as my knowledge goes for me. Pierre should be back on line before too many days go by and I am sure he will have a much better answer than I can give you. I hope the new meds work to get your BP lower for you. Guest guest
I'm taking 450mg/day of Avapro (ARB) first thing in the morning, as well as 900mg of Labetalol (Beta Blocker) 3 times/day (2700 mg/day). I had some problems at first with dizziness, and from time to time I can get about 5 seconds of disequilibrium. However, my neph says that my progression has stopped at about 35% GFR (4 years after diagnosis). My BP stays right within a nice range (120/70) to (130/80). I tell everyone that I know if I wasn't taking the pills -- I would probably not even know that I had the disease. One thing -- I'm still taking lots of other things, like diuretics, nephrocaps, calcitriol, sodium bicarbonate, iron, and procrit. But the two big BP medicines are the biggest part of my treatment... I have proteinuria (about 800mg/day), a lot of microscopic hematuria and hyaline casts. Neph has added a beta-blocker, Tenormin 25mg/day (atenolol) to bring down my heart rate. I request members of this egroup to kindly share any experience with ARB - Beta Blocker combination. Guest guest
Hi Amy, That is good you have been able to control your BP. It is vitally important to protecting your renal function because elevated BP by itself is a risk factor for kidney failure. I hope this combination of BP meds continues to be successful for you! Guest guest
Hi Ram, Sorry it's taken me so long to respond to your post. I was trying to locate a post I had written for someone here on the difference between an ACE and an ARB. I finally found it ...July 17th 3:00pm or so, Subject is Protein levels and Ramipril. May want to read this post first then come back to this note. (I'm trying to condense, which is so hard for me...(hehehehe). Ibesartan is the same drug as Avapro. Tenormin = Atenolol which is a beta blocker which,as you said, lowers your heart rate. Beta blockers decrease the influence of the sympathetic nervous system (your fight or flight response) on the excitability of the heart, cardiac work load, oxygen consumption and the release of renin. In this way, beta blockers decrease heart rate, decrease blood pressure and improve some fast heart rhythms. They (the entire class of beta blockers) have been proven over and over to decrease morbidity and mortality in patients with heart disease, and the potential for those patients at a higher risk. One of the biggest things that the nephs look at with renal disease is not exactly how long it will take you to get to end stage renal disease, but the complications that also go along with renal disease including hyperlipidemia, hypertension etc. As one of the local nephs I've been working with said 2 weeks ago, " the idea of " dying " from kidney disease in the long run is much less likely than having a cardiovascular event " (atherosclerosis, heart attack, stroke etc). In other words if you have kidney disease, you will much more likely develop cardiovascular problems way before you develop ESRD (end stage renal disease). That is why the nephs and IM docs (and NP's hehehe) work soooooooo hard on getting everyone to goal B/P of <130/70, and decreasing the lipids. If your interested in the new guidelines for CKD lipids for adults, go to my post 7/21/04 the link for the new guidelines are there. Let me know if you have any problems navigating there. Hope that helps, if not clear, just email me back and I try to explain things another way. Guest guest
Hi Ram, I don't know about ARB, but I have been taking beta blockers and they really got my BP down well. I have also taken atenolol for many years for an irregular heart beat and it works really well. My neph took me off atenolol for a short period and my pulse rate went up immediately. Good luck and stay well!.........Gloria > Hi > > I have been taking 300mg of ARB (Irbesartan) > but I had a BP reading of 140/88 with > a heart rate of 104 at neph's office. > > I have proteinuria (about 800mg/day), a lot of > microscopic hematuria and hyaline casts. > > Neph has added a beta-blocker, Tenormin 25mg/day (atenolol) > to bring down my heart rate. > > I request members of this egroup to kindly share > any experience with ARB - Beta Blocker combination. > > Thanks > > Ram
