I am no expert, but was under the impression that is was predigested and therefore you don't need the enzymes. (I also understand it is NASTY, which would not be an issue for Gtube feeds, of course) Again, I am not an expert on this, but seems like I have heard this several times. We used them down Eme's Mickey tube and didn't have any problems once we got our very " non " standard routine down. Ask your doc about using these and then if you have questions on how to get them down email me and I'll tell you about our odd but functional way of doing this. good luck! I could be wrong about that but that is the impression 's drs gave me. There was some question as to whether or not she needed enzymes, but now it appears as though she does. Everyone on the list seems to have such good things to say about it and if it is easier to get through the tube that would be so wonderful. The formula is more broken down but still needs enzymes to absorb most of the fats in it. There are several elemental formulas/mixes that are elemental that do not require enzymes, very expensive and not covered by most insurances. We have the exact same issue - fortunately, they are still able to use the viocase and are gaining weight well. However, since the enteric-coated enzymes are so much better than viocase, we tried them on a new formulation from Pancrecarb. It is Pancrecarb MS-4. It is a smaller bead and unlike the creon or larger formulation of pancrecarb, it does go down the tubes. It's a bit of a struggle even then, however. We found that Mick's 16 French button handled it better than Alli's 14 French button, which is the smallest that will allow the enzymes to go down. Here are some tips I managed to come up with - 1. You need to mix your formula with apple juice, and the amount depends on how many capsules you'll use and what formula your child is on. They didn't test it with pregestimil, which is what our kids are on, but we used the same ratio as prescribed for another formula that is partially digested. 2. I found that warming the apple juice/formula mixture works better than trying to mix the enzymes in cold. Warm formula/apple juice seems to loosen the enzymes a bit, though they'll still clump at the base of the tube or at the tip of the feeding syringe. 3. When the enzymes clump at the tube, where it meets the button, I massage it a bit, then draw the syringe plunger back up to draw up a bit of the formula and some stomach juices. If you can get the stomach juices up, they kind of fizz and help loosen the enzymes, and then you can push it back down the tube. This usually does the trick, though sometimes you have to do the pull - push thing a few times. We've gone back to the Viocase because it was just too stressful to do the other enzymes, but I'd put up with it if the kids weren't gaining weight on the viocase. Does your daughter go to a cf clinic? if so you can call the dietician or the gastroenterologist that covers the cf kids and they will know. It seems to me that one of the formulas is blue colored, because we joked about it being smurf soup. Guest guest Posted April 18, 2001 Report Share Posted April 18, 2001 My daughter Sydney won't eat either, but she is on Peptamen Jr. (predigested formula) and has been off the enzymes for some time, and is gaining weight. Peptamen is expensive, but our state's medicaid program covers it. The doctor wants us to put her back on enzymes, but while she was on Viokase she had some bowel problems and elevated lipase levels, so we are concerned about going back on. I'm encourage to hear people are putting enzymes down g-tubes though, as the doctor told us we couldn't do this without clogging it. Syd will not take the enzymes orally. Has anyone put creon through a bard g-tube (24 french)? I know there's protocol for doing it with a Mic-key tube.
