I have to get labs drawn this afternoon for my upcoming 6month checkup at PB next week and I lost the lab request form... I have asked PB to fax me a copy twice, but they never have... and I am running out of time for kaiser to process the labs and so I can get a copy to PB on the 8th... Does anyone have a lab form sitting around that they could write down what I need drawn? I have an extra lab form from my primary so all I have to do if bubble in the needed labs.... so I wouild greatly appreciate if someone can help me with what I need drawn...
