is 10 and has been receiving Beckett for seven years. It

serves as his secondary insurance and covers everything and anything our

primary insurance denies. We live in Wisconsin. I believe there is no

limit to the number of children the program covers, you just have to meet

certain criteria. Because still gets tube fed twice a day he still

qualifies. We meet with a rep every two years to determine eligibility and

have to fill out paperwork the other years. I know quite a few children

who receive that also just for asthma issues (I'm a schoolteacher).

We have a lottery system in IL. I don't know the number of families served.

You put your name in the lotto. If you win, you get the extra financial

assistance forever. You do have to have an eval every 2 yrs to prove that your

child still qualifies. The montly stipend is $564. It has increased yearly

from $525 when we started a few years ago. The family can use the money as they

see fit. At the reeval time, they also send out a voluntary questionnaire

asking how that money has helped meet your child's needs and an affidavit that

the money was in fact spend on issues related to your child. That can include

making up for lost income when parents are unable to work to the extent that

they did before. We were very fortunate to be drawn immediately after putting

our names in.

I think those funds come from the Beckett Waiver. There are also funds

available for respite, home nursing, modifications to your home, etc that are

based on income and asset requirements of the family. In order to get that

assistance when Aubrie was an infant and we needed home nursing, we liquidated

all of our stock assets and put the cash into the house because your residence

is not considered an asset. However, I think there are now rules about that --

maybe a time frame in which you have to wait so long after making that sort of

transaction. I don't know for sure since we're not utilizing those services

now.

a version of the Beckett waiver but it only serves 200 kids,

with half of those slots reserved for vent dependent kids.

I have friends that are in the process of moving to Idaho because

they offer the waiver to all children with disabilities. I understand

Idaho's waiver program serves upward of 5000 kids.

So my question's are...

Which State do you live in and do you recieve a Medicaid Waiver? Does

the waiver serve large or small numbers of children, and also does

the waiver include such items as respite, diapers or what exactly

does the waiver mean to you.

A quick update on Nicolas. He is now 6 1/2 and is in Kindergarten.

The year started kind of Rocky with him needing 2 assistants at all

times during school...he is now down to mostly 1. He attends a half

day at public school and a 1/2 day at a private school we pay for. It

is a good mix for him.

Here in Florida, the Beckett Waiver is called the " Model Waiver " . It is

very limited to scope and number of children. Here are some snipped pieces of

the waiver....below that is the amount of children allowed at one time....

DESCRIPTION OF THE SERVICE

The Model Waiver, formerly known as the Beckett Waiver, is a special

Medicaid program

approved by the Centers for Medicare and Medicaid Services (CMS) and operated

by the

Florida Medicaid Home and Community-Based Services office and the Department

of Health

office of Children's Medical Services. The Economic Self-Sufficiency Program

of the

Department of Children and Families coordinates the eligibility determination

upon receipt of a

referral from the Medicaid Home and Community-Based Services office. Services

that are

offered in the Model Waiver may include in-home care that must not exceed the

comparable

Medicaid cost for hospital care. The intent of providing services under the

Model Waiver

Program is to delay or prevent institutionalization and allow eligible

recipients to live at home in

the community as opposed to placement in a hospital. Services provided under

the Model

Waiver Program are limited to assistive technology and service evaluation;

environmental

accessibility adaptations and respite care.

ELIGIBILITY

The Model Waiver program serves a limited number of children: Pre-application

criteria include

the following six elements. The child must:

· meet disability criteria for Social Security Disability;

· have a level of care determination of " at risk for hospitalization " ;

· have a diagnosis of a degenerative spinocerebellar disease which are

generally

identified in the 330-337 range of ICD9-CM diagnostic classifications;

· be under 21 years of age;

· be able to be maintained safely in the home using home and community based

services;

and,

· not be eligible for Medicaid when the income and resources of their

parent(s) are

considered and would be eligible if the children's own income and

resources are within

Medicaid limits.

The area Medicaid office will take the lead in initially screening for the

Model Waiver Program.

If the child currently has Medicaid that makes him/her ineligible, the CMAT

cannot staff the child

for waiver services.

If it is determined during the screening process that the child is not

eligible for the model waiver,

it may be appropriate to offer the family a continuity of care staffing to see

if the child is in need

of other services and to aid in coordination of services.
