Yesterday started experiencing some lung pain and my inspired volume was down as were sats on exertion. So, being Sunday and partying hard with all of you, I just took some vicodin not for the feel of the pain but to ease it on the lung so it would function better. It worked to get me through until today when I could call the pulmonologist who then sent me to the ER, but with specific instructions of what they should and shouldn't do.He was afraid of blood clots and pneumonia. Well, turns out chest x-ray showed mild pneumonia and also d-dimer blood test was elevated. I can't have a CT with contrast due to creatinine level so had a VQ scan. It turned out ok revealing no blood clots. So got a nice iv of levaquin there and more to bring home and told to take it easy 2-3 days. So all is fine except for spending 7 1/2 hours at ER. Guest guest Posted January 1, 2008 Report Share Posted January 1, 2008 Well, as I'm sure is not surprising I am even a bit obsessive over watching my oxygen and using my spirometer and other breathing gadget and the treadmill. So, in addition to the pain I was able to tell the doctor the effect on my inspired volume and the effect on my sats on the treadmill. So instead of just pain I had two objective measurements. Therefore, I went to ER with clear instructions of what they should and shouldn't do or it would have been longer than it was. For instance, I had the Chest X-Ray and hour and a half before I ever got taken to an ER room and had I not know about a d-diner when they took the first blood they might not have tested for it, but they did and had that result waiting when the ER doctor first saw me. My only complaint is that I just don't equate the word " Emergency " with 7 1/2 hours, about 2 1/2 before being taken back even. Then I would have had to have struggled to see if their tv system would have let me type here....lol Also, for practical purposes, I want to hospital in January. December 31 was fine as I'd met my annual maximum. February 1 I'm on medicare. January I have one month of all new deductible and copay on my current insurance so staying would have cost out of pocket probably $1750. Had I been one day later going would have. Because just the nuclear testing uses up copays in a hurry. Also, just for a fleeting moment I had a horrifying picture of me running around the house just wearing socks and a scarf, but I realized you didn't just mean them. However, is there something special about watching the neck and ankles or is that just a phrase? With warm wishes from you, I'm warm all over. I did turn the heat up though last night above my normal just as a precaution. My thermostat on my body (wonder if I can get a new one at NAPA) went out sometime back and I tend to keep things cooler than I did before. Of course, I'm also experiencing the lowering of normal body temperature so I'm sure thats perhaps related. I don't even understand what my normal temperature is anymore. I remember when young mine was like 99.1 as opposed to 98.6. Now its lower than 98.6 but it fluctuates so much I don't honestly know whats normal and what isn't for me anymore. I think its around 97.0. So does that mean 98.4 is now running a fever. Darn, even temperature has gotten complicated. At least I understand oxygen levels and blood pressure and know whats normal there. Guest guest Posted January 1, 2008 Report Share Posted January 1, 2008 Well, my ER doctor was incredible....thorough, knowledgeable, maybe the best I've ever had in ER and she immediately called pulmonologist to discuss next steps. It's the triage nurse who was the pain. I did ask him to see his medical license. " Oh, I'm not a doctor....I never claimed to be. " So, I said, " Then why are you acting like you know more than my doctor who does have one. " Actually, the nurse who I first saw in ER said I should report him. Ah, does that give you a clue when his fellow employee doesn't just apologize but encourages you to report the guy. lol. I know i'm stupid.....next time to ER triage, I'll be clutching my heart and my level of pain on 1-10 is 97. lol. Of course you mention heart and you're going to be admitted so didn't want that. Actually, one thing I do forget is that my pain tolerance is so high I really do need to translate to normal. We're not sure why but may be related to other neurological issues. My doctors know but others don't. My physical therapist actually has measured it. Using neuroprobe when I said I was mild pain, she measured record levels of pain for her office. Actually, my ex-wife and I talked about that while sitting yesterday, as all those kept moving in front of me. Had I simply called for an ambulance and had one taken me, bet I'd have been out in half the time. As I understand it, a negative D-dimer test will virtually rule out thromboembolism. That is below a certain level. Once above that at each level it may show an increased probability but is influenced by other factors. But an above range number does not definitively say you do have clots. In my case my D-dimer was three times the level of ruling out but only about 30% above a " normal " range. Ask your doc about keeping Levaquin (or another ab) on hand for when the infection starts. My doc gives me a script for a year at a time and lets me make the decision. He tells me that I am smart enough to know when I am sick.....and I am. Pneumonia left untreated even a short time can be devastating. We do not need new scar tissue. You are so smart to act quickly. It is so easy to dismiss pain and feeling ill. Be well. After 7 1/2 hrs. in ER you'll need a rest. Or, maybe you got plenty of rest...waiting, although not pleasant rest. It's always a scare when one of us has a flare of whatever. Also, not sure just the pain would have pushed me by itself as quickly as the other indicators-my spirometer and breathing exercises and my oximeter on exertion. Probably another lesson learned too that the spirometer not only is good to exercise but also a good check on your breathing. I also knew that when your inspired volume drops to 1000 the risk of pneumonia jumps up and mine did. Now I did quickly get it back up by taking vicodin. One thing I learned from VATS recovery in hospital was that even when you think you're fine with the pain, your lung may not agree and may let it prevent it from doing more. Amazing, one vicodin and I went from 1000 to 1500.
