Hi and Happy Happy to all,

Here is a link to an article about the inconsistency in treatment

regimens of PF patients after DX. As Bruce so loves to remind us and,

he is so right, WE are in charge of our healthcare. My Pulmo considers

me to be a pain in the backside ... my PCP however wishes all his

patients were like me.

Very interesting article. What I find interesting too is that with

the inconsistency I'll bet lots of those pulmonologists are still

adamant over their way. At least mine said, some believe this and

some believe that and talked about the options although not sure he

understood my choices.

Well, my pulmonologist probably believes I'm a pain. My PCP knows I

am but he shows me respect because he knows how many times when I've

felt there was a problem there turned out to be one. He didn't

believe (and he basically admitted it later by not disagreeing when I

said it) that I had a problem beyond being out of shape, but he still

humored me and took precautions and checked. It just took one six

minute walk and he realized I was right, that there was something

else wrong.

Schools here are increasing courses on talking and listening to

patients and encouraging doctors to take continuing credit courses in

the area, recognizing how much failing to do so well impacts their

treatment.

While you wait for Bruce's scholarly reply I'll put in my notions since I'm thought to have BOTH UIP and Fibrotic NSIP.....now

ILD stands for Interstitial Lung Disease & NSIP is but one of those.....confusion arises because different Pulmos (either in the same country as well as in different countries) use intermingling terms for the whole bang lot. I wish they'd have an International Symposium & sort out the nomenclature once & for all...an International Standard Glossary for Interstital Lung Disease!

The other term that get s confused is IPF: Idiopathic Pulmonary Fibrosis. In some places it means the same as UIP (Usual Interstitial Pneumonitis) . Some doctors now use the term IPF ONLY to mean what others call UIP!!!! HOW confusing is that.

In Australia IPF is still used as an UMBRELLA TERM. UIP & NSIP, BOOP , & others are used to indicate the SPECIFIC FORMS of IPF.....they are ALL Interstitial Lung Diseases! (ILD's)

With NSIP (Non-Specific Interstitial Pneunonitis) there are 2 Forms: Fibrotic & Cellular. Cellular has a much better prognosis than Fibrotic Forms. Cellular NSIP is the one that responds best to Steroids. Fibrotic NSIP is still treated with Steroids but the results are not as clear-cut. Relapse is upwards of 60% according to my Lung Transplant Specialist.

It is possible for lungs to have BOTH Fibrotic NSIP AND UIP at the same time although no one has worked out why as yet. When this is the case the UIP seems to 'take over' as the dominant disease process.

There are thought to be differences in the way Fibrosis gets started bewteen UIP & NSIP but it IS all still THEORY!

ILD is Intersitial Lung Disease and and PF is Pulmonary Fibrosis so

basically the same. Among those there are over 200 specific diseases.

NSIP is one. It is Non-specific interstitial pneumonia and is the

second most common form behind IPF (Idiopathic Pulmonary Fibrosis).

It does have a better general outlook in terms of lifespan and also

does seem to respond more often and better to medications.

As to the GERD, weakness of the lower sphincter is common. I have the

same as the acid has damaged it. Now that makes the reduction of acid

through medications even more critical because the lower sphincter

lacks the strength to keep it from rising. You need the meds to

protect your esophagus and lungs. As to the esophagus, one thing that

does need to be watched carefully for is Barrett's Syndrome. It is

not a serious problem itself but it often a precursor to cancer. So,

you need to do everything you can to protect your esophagus and

lungs, including meds, raising the head of your bed, whatever it

takes. You can't undo anything but just fight to keep it from

worsening.

As to the stages of GERD, I rarely hear them referred to but here is

what I found:

The first stage is simply excess burping. As you digest your food,

gases start to build up. If any of the gases escape through your

lower esophageal sphincter they can result in excess burping which

can seem innocuous enough but can lead to the more common stage 2.

The second stage of acid reflux symptoms is what you hear about more

commonly. It is a severe burning sensation in the chest (specifically

the breast bone region). It is due to the stomach acids escaping the

stomach, passing the lower esophageal sphincter and moving up to the

chest region. Because this liquid is very acidic, it can create some

very severe pain. Many people even mistakenly believe they are having

a heart attack when they are struck with this acid reflux symptom.

The third stage is a sore throat. Most people assume that all there

is to acid reflux is stage 2. In actual fact, the stomach acid can

pass up into the throat doing damage (sometimes permanent) to the

throat. The affected person may believe that the sore throat is

actually caused from something else, further clouding the issue.

The last stage of acid reflux symptoms is a sour taste or even a

severely sore mouth. Similar to stage 3, the acid actually makes its

way all the way up to the person's mouth again causing damage which

can become permanent.

Now just because stage 4 happens sometimes doesn't mean thats where

you are at all times. And you can be reduced from it back to stage 2

often. However, with any the key is removing the acid. It can be

treated more than the reflux itself.

The other term that get s confused is IPF: Idiopathic Pulmonary Fibrosis. In some places it means the same as UIP (Usual Interstitial Pneumonitis) . Some doctors now use the term IPF ONLY to mean what others call UIP!!!! HOW confusing is that. In Australia IPF is still used as an UMBRELLA TERM. UIP & NSIP, BOOP , & others are used to indicate the SPECIFIC FORMS of IPF.....they are ALL Interstitial Lung Diseases! (ILD's) With NSIP (Non-Specific Interstitial Pneunonitis) there are 2 Forms: Fibrotic & Cellular.

Cellular has a much better prognosis than Fibrotic Forms. Cellular NSIP is the one that responds best to Steroids. Fibrotic NSIP is still treated with Steroids but the results are not as clear-cut. Relapse is upwards of 60% according to my Lung Transplant Specialist. It is possible for lungs to have BOTH Fibrotic NSIP AND UIP at the same time although no one has worked out why as yet. When this is the case the UIP seems to 'take over' as the dominant disease process. There are thought to be differences in the way Fibrosis gets started bewteen UIP & NSIP but it IS all still THEORY! Thanks for sending that Link , I found it most interesting. I've just been reading a Paper from Dutch Researchers published in 'The Journal of Nuclear Medicine', albeit from 2005. These people were trying to find a means of PREDICTING success with Predisone using something called Ga Scintigraphy Tests...they failed BUT it was their conclusion that caught my eye: " Pulmonary Ga uptake cannot be used to predict response to corticosteroid treatment OR prognosis in patients with IIP (Idiopathic Interstitial Pneumonia). Apparently, the inflammatory process influenced by treatment with methylprednisolone DOES NOT DETERMINE THE PROGRESSION OF THE DISEASE. This finding supports the hypothesis that although inflammation is present in IPF and NSIP, it is neither the HALLMARK of the disease NOR the major factor determining prognosis."

Di, That's what this board is here for. No question is dumb. There is so much to learn about pulmonary fibrosis. Stick with us and ask any and all questions. We're here to help as best as we can. (ILD's) With NSIP (Non-Specific Interstitial Pneunonitis) there are 2 Forms: Fibrotic & Cellular. Cellular has a much better prognosis than Fibrotic Forms. Cellular NSIP is the one that responds best to Steroids. Fibrotic NSIP is still treated with Steroids but the results are not as clear-cut. Relapse is upwards of 60% according to my Lung Transplant Specialist. It is possible for lungs to have BOTH Fibrotic NSIP AND UIP at the same time although no one has worked out why as yet. When this is the case the UIP seems to 'take over' as the dominant disease

NSIP is one. It is Non-specific interstitial pneumonia and is the

second most common form behind IPF (Idiopathic Pulmonary Fibrosis).

It does have a better general outlook in terms of lifespan and also

does seem to respond more often and better to medications.

As to the GERD, weakness of the lower sphincter is common. I have the

same as the acid has damaged it. Now that makes the reduction of acid

through medications even more critical because the lower sphincter

lacks the strength to keep it from rising. You need the meds to

protect your esophagus and lungs. As to the esophagus, one thing that

does need to be watched carefully for is Barrett's Syndrome. It is

not a serious problem itself but it often a precursor to cancer. So,

you need to do everything you can to protect your esophagus and

lungs, including meds, raising the head of your bed, whatever it

takes. You can't undo anything but just fight to keep it from

worsening.

As to the stages of GERD, I rarely hear them referred to but here is

what I found:

The first stage is simply excess burping. As you digest your food,

gases start to build up. If any of the gases escape through your

lower esophageal sphincter they can result in excess burping which

can seem innocuous enough but can lead to the more common stage 2.

The second stage of acid reflux symptoms is what you hear about more

commonly. It is a severe burning sensation in the chest (specifically

the breast bone region). It is due to the stomach acids escaping the

stomach, passing the lower esophageal sphincter and moving up to the

chest region. Because this liquid is very acidic, it can create some

very severe pain. Many people even mistakenly believe they are having

a heart attack when they are struck with this acid reflux symptom.

The third stage is a sore throat. Most people assume that all there

is to acid reflux is stage 2. In actual fact, the stomach acid can

pass up into the throat doing damage (sometimes permanent) to the

throat. The affected person may believe that the sore throat is

actually caused from something else, further clouding the issue.

The last stage of acid reflux symptoms is a sour taste or even a

severely sore mouth. Similar to stage 3, the acid actually makes its

way all the way up to the person's mouth again causing damage which

can become permanent.

Now just because stage 4 happens sometimes doesn't mean thats where

you are at all times. And you can be reduced from it back to stage 2

often. However, with any the key is removing the acid. It can be

treated more than the reflux itself.

My Tai Chi instructor always taught me that the only stupid question is the one you don't ask. You can pretty much garantee that if you ask a question, no matter how minor it might seem to you, someone else is wanting to know the answer too. I You will never make a fool of yourself by asking a question, and I haven't come across anyone on here that will belittle you for asking. Some questions that others have asked on here are ones that I haven't even thought of, but reading the question, and the subsequent replies, I have found out that they are very relevant to me.

I genuinely believe that the more knowledge we have about our condition the better quality of life we will have, and if that involves learning from someone else, that's fine.

Getting an answer to a question can be the single flame in the dark. Please don't be afraid to ask

Tai Chi.....not for me. Maybe Mai Tai. Not only is every question

welcome and perhaps I do like the challenge but I can assure you I

learn far more here every day than I share and I thought 's

answer was excellent and nice to see an overseas perspective. And,

she did point out the evolving confusion. Actually the various forms

here are individually identified by specific pathologies as among the

6 or so most common ILD's there are definite distinguishing

pathological features.

I learn not only from the answers of others but from continued

research before I answer any question. As I double check my answers,

I often find one additional piece of information I didn't know.

I've also asked more than my share of questions, many about the

practical issues that the experienced here long ago worked through,

whether it be type of oxygen delivery systems, cannulas, oxyview

glasses, humidification, water traps, traveling, or who knows what.

There are still thousands of aspects of this disease I've yet to

experience. I also gained input into VATS from those who had actually

had them to combine with my doctors' input before having mine and

have listened and read comments on prednisone to reconfirm in my mind

that I've made the right decision for me.
