Guest guest Posted July 29, 2003 Report Share Posted July 29, 2003 My docotr had to fill out a section of a form for funding for me. The section was Doctor's Statement in big bold cap letters. In the section the doctor put Sarcoidosis and then Permanently disabled and he had to sign it. Now I am wondering if this will be sufficient to present as new evidence to get a second hearing. Guest guest Posted July 30, 2003 Report Share Posted July 30, 2003 My doctor filled in the same thing Sharon. It said "CNS Sarcoid" "Palatal Myoclonus" and "Diabetes Insipidus" and "Other" LOL. He also ticked the Permanently Disabled box. Which I hated seeing but I got disability first go. Good luck. He also ticked the Permanently Disabled box. Which I hated seeing but I got disability first go. Good luck. I really had to fight the one company to right the wrong of a DME that should not be allowed to be selling chairs. They still are in business. I don't know how.
