Prostate cancer classification

The patient was found to have a high-grade T3A, N0, M0 transitional cell

carcinoma of the bladder, as well as having prostate cancer detected in both

lobes. He had a Gleason " 3 plus 4 " disease in the left and " 3 plus 2 " disease

in the right.

Should the Gleason scores be written as 3 plus 4, 3+4, 3+/4, 3/+4? Same for the

3 plus 2.

Thanks y'all!
