Guest guest
Female patient with hx of breast CA and ESRD. In the assessment, the doc says: " This is a 62-year-old African-American female, severely ill secondary to metastatic prostate cancer. She also has end-stage renal disease on hemodialysis on Monday, Wednesday, and Friday. " Ummm, did I miss something here? Shouldn't the doc be saying hx of metastatic breast CA instead of prostate CA? Should I change it or flag it? He is not sloughing the word at all, he is one of those blue-moon perfect dictators that we all dream about. :-) Diane MT forum administrator http://mtgripe.proboards.com/ * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * *
Guest guest
Now, at the end of the report, the doc says, in the plan, " concerning her metastatic pancreatic cancer " but he stumbled over the word and started to say " prostate " , but changed it to pancreatic cancer, of which the pt. also has a hx. So, I am going to change the prostate to pancreatic. Thanks all! Guest guest
Hi, Marsha here. I think that this is judgment that is crucial at this time. I would say breast CA in the part of the dictation that specifies prostate cancer. I think the good doc got mixed up with another patient he had been dictating on. Other than that, I would flag it if you are not sure. Marsha Stat H & P help > Female patient with hx of breast CA and ESRD.
