Help again

Hi,

Need help again. I promise not to bother you as much

when I get all the books I ordered in the mail.

Here are two words in " " that I can't make out. Please

help if you can.

2) Depression and anxiety. Patient has been tried on

Paxil, Prozac, Zoloft, " Phrasont " , and Xanax without

any relief. Prozac just made her sleepy.

PELVIC EXAM: VVS negative. External genitalia on the

left " mon " .

Patient does have a 1-cm lipoma,nontender, mobile,no

increase in size.

Also I don't have my Lab & Pathology book yet. Can

someone give me a sample of blood work up for a person

that has sclerosis of the liver? I am not sure of any

terms in " " .

I am trying to transcribe my first lab ever without

any lab example. I know I am terrible, but I

did order the Stedmans Pathology and Lab book.

Mrs. X is a 48-year-old woman who comes to see me for

followup.

She is still noncompliant with " Lasceros " . She only

takes it b.i.d. She states that her last lab tests

showed that her " ung " is 73. It was up as high as 93

and at one point it went down to 86. Now it is 73. Her

" FSH " done on 12-04-01, is .04, which is normal.

Patient is chronically anemic with a WBC of 3.0

" HMH " is 9.2 and 29.5. Platelet is 117. per cu mm.

This is most likely due to her sclerosis of the liver.

Good " PTINR " . Protein of 15.3 " INR " 1.2 " THH " is 3.65.

Patient also needs a refill on her Soma 350mg. 1PO

q.h.s. prn.

Patient is also requesting a Flu vaccine.
