Patient has Hyperthyroidism, Gastroesophageal reflux

disease, Hypertension,and Hypercholesterolemia.

s/l Hematid-LA

Paitent has Sinusitis, Allergic rhinitis and

Gastroesophageal reflux disease.

s/l Triamsin 1.01% ointment

Patient has Psoriasis (knees, elbows, and ears)

Don't know if you got an answer yet, but the last one could be triamcinolone

0.1%.

Learning the meds were the worst thing for me when I started. It does get

better.
