I think the first is probably supposed to be E & M for Evaluation and Management instead of E/M. But the second is beyond me *POV* See no. 2 under P. Thanks for any enlightenment. Pt. for F/U left hallux which is 1-month S/P P & A matrixectomy. Pt. C/O long, thick nails with P on A. Pt. states no C/O left hallux. Exam reveals no change in neurovascular status, B/L All nails are elongated, hypertrophic, dystrophic and yellowed with crumbly S/U debris with dark discolorations. There are multiple hyperkeratotic lesions noted sub 2. 3 and 4 B/L. The left hallux lateral border is seen with normal healing with no signs of cellulitis. A: 1.Onychomycosis x10 with DM PVD. 2. Hyperkeratosis x6 with DM PVD. 3. One month S/P P & A matrixectomy, left 1st. P: 1. Aseptic onychoreduction x10 OM nails. 2. E/M left hallux POV 3. SD lesions iwth aftercare. R/A: Two months or PRN.
