Anterior chambers are deep and clear. Pupillary reactions are normal.

Dilated exam shows right eye with pseudophakia. Posterior chamber

and ___(s/l F1) is well positioned. There is some very early

peripheral _______( s/l cat sellar) fibrosis.

Any help would be greatly appreciated.

Could it be Skelaxin, a muscle relaxant?

If so, I would type Skelaxin samples at her request, #36, one to two, three to

four times per day.

Maybe: Skelaxin samples at her request 1-2 tablets 3-4 times a day. (the

one to two being the number of pills to take 3-4 times a day). I don't know

if you can add the " tablets " in there, but in this case it would definitely

help. **not sure what the #36 means though. Hope this helps some.

Probably Skelaxin and I would put 1-2 to 3-4 times per day #36 given.

The reason I went with one to two, three to four times a day, is because

Skelaxin comes in 400 mg

tablets, and the QLDB says the dosage is 800 mg three to four times a day. I

guess she could make it

one to two tabs, three to four times per day. I don't know how much she is

allowed to change.

I think the #36 is the number of pills dispensed.
