In Colorado, there is a State form which one gets from the dr., and then either a necklace or bracelet can be ordered. These are to be worn at all times, and all emergency and medical personnel are trained (we hope!) to look for them. Since it is an official state program, it removes the element of doubt in how to handle anyone wearing the necklace or bracelet.
