Important Announcement - 2001 Database Closure

This is to notify you that the deadline for the 2001 reporting period has

been extended and will close on April 4, 2003.

You may submit your data using the modem (Modems have been operational and

accepting files submission since January 17, 2002) or on-line (if you have

completed a packet and we have activated your account). Please contact my

office for assistance if you have any questions.
