You are using your ingenuity to create something for which there may be a

market, and that is what this Great American experiment is all about.

Actually, now that you mention it, I am working on a device that will

measure epididymis wedge pressures to assure that all field personnel remain

mentally and physically well. I am not sure whether to put a numerical

(digital or analog) gauge or simply a light to indicate when epididymal

wedge pressures exceed the recommended threshold and need release.

I'll have to agree with Terry. I know some " snotty-nosed LP's " straight out of

paramedic school with no street experience who could get some use from a gadget

like this. Not all of us were taught on a monitor that counts the QRS complexes

for you (by the way.... when did they start considering this an acceptable

method of counting a " pulse " , when in fact, it's just a measure of the

ELECTRICAL activity, not the actual generation of a pulse)?

Yeah, I beat my students up about excessive reliance on techno-gizmos, (pulse-ox

and Doppler come to mind) but I can also remember a time when EKG's were

considered an unattainable luxury.

I don't necessarily see this device as a solution to any of MY problems, but at

least he is analyzing, adapting and thinking of potential solutions. The good

ol' American marketplace will prove the pudding. If it fills a need they will

buy......

And look at the latest batch of techno-goodies.......recent studies seem to

indicate all these public access defibrillators only achieve a 1% increase in

survival.......

Even tho this is but a small increase, I hope I'm near one when my turn

comes.......

I only put the timer together because I wanted one but there wasn't one

available. I mainly respond to calls at night from my house and either

forget my watch in the rush or end up on a MVA out in the woods in the dark.

Allheart offers a pulse timer and that was a watch that mounted to your

scope, I bought one of those and did not like it.

My squad and the squads local in my area use them and find them to be

helpful. Up to now I have only sold them be word of mouth.

I am also working on a system to work with our radio to stop the feedback

You see...this is where my epididymal wedge pressure monitor comes in. Too

frequent unilateral relieving of epididymal wedge pressure can have a

negative effect on vision.

I was watching a special on the history channel a few days ago and they said

the Brontosaurus, Tyrannosaurus and the epidimus were extinct and had been

for millions of years. I can't figure how this wedge you are talking about

will help resurrect them. And what is all this about negative visions?

Sounds drug-induced to me. Do you have too much time on your hands, or is

Norton Antivirus scrambling your messages so the Soviets can't detect them?

I know you joke, but I've read articles where plethysmographs were used in

the prosecution of a select criminal set. With all the rage on " mercury

devices " for sensing vehicle movements, I'd hate to think about the

application of this technology in the ambualnce. Can you imagine how many

" registers " crews participating in SSM would get vs those who are allowed

station time?

And you are correct. The monitor does NOT give a pulse rate. Palpable

pulses are just that - palpable. The closest machine reading would be aan

oximeter, and even that's not the same as a palpated pulse. Anyway, a pulse

vital sign has a location associated with it. What's the location for the

pulse measurement on an ECG (where is it FELT?)? Good point...

I have been doing this a long time I am not a new boy. Over the years I have

noted that a cardiac monitor a pulse oxcemeter and my own pulse count do not

always agree, so who or what would you rather trust if it was your life?

Mr. -

I trust an experienced, prudent practitioner, such as yourself.

Instrumentation is usually a pretty good indicator of general status - When you

explain your findings and actions to your Medical Director, or go to court,

however, you better have taken an occasional pulse.......

Machines lie more often than we would like to admit, and the prudent

practitioner will not overly rely on just one assessment methodology.

What worries me about a watch is that I have seen so many Paramedics and

EMT's that do not wer watches when they report to work. They have grown so

accustomed to using the Pulse OX and or the EKG to obtain the pulse rate. I

have a great deal of concern with this practice. I am a firm believer that

regardless of what the monitor or pulse ox reading shows one should always

take a pulse manually, using a watch. Hands on to me is so much more

important than relying upon machines to do our job. I have been question in

the past as to why I am taking a pulse using my hand and watch, when the

task has already been done with the pulse ox. You know sometimes pt.s with

A-Fib will really jack with the pulse ox machine. So I guess being from the

old school hands on is the way I choose, plus it just seems to be so much

more personal in establishing a rapport with the patient. Machines are great

but they have know to fail. Just my opinion and my preference on taking a

pulse and counting respiration's. I just wish more EMS personnel would wear

watches to work.

Being one of those lucky people who is allergic to some metals, I do not

wear a watch. However, my solution is a pocket watch. Really handy, and

because they have clips for attaching to belt loops or chains, they can be

carried wherever it is most convenient. Available at Wal-Mart for $10.00 or

less. At that price, it might be feasible to put 2 or 3 on each ambulance.

Attach them to items that the crew usually takes with them to the patient

(oxygen bag, monitor, etc) and you always have a watch handy.

You are absolutely right about doing a hands-on pulse, even with the monitor

and pulse ox. If what you are feeling matches what you are seeing (one

palpable pulse/one complex on the monitor) then you may be able to use the

rate showing on the monitor as the pulse count, but nothing can replace

actually feeling the pulse. Even if the heart rate on the monitor does

match the pulse rate, you still need to assess strength and quality of the

pulse.
