Patient Response Forms

There are pros and cons about the benefits of patient response forms

or patient service survey/satisfaction forms. I'd like to get your

ideas.

I would appreciate receiving a copy of the tool that you use to get

patient feedback about the service she/he received from your EMS.

What other methods do you use to gather patient opinions?
