Is there an advantage to having your own? Perhaps a link to the UNOS calculator with an explanation of the values, etc. would be easier to maintain? Just curious. I checked both mine and yours, and they both seem to give basically the same answers when I put in low numbers but they get off from each other as the numbers get larger.... still close but as they started getting up toward a MELD score of 20 the numbers were far enough off that they'd round to a different number. It looked like there was a difference in how they handled the INR number, but I didn't look far enough to figure out what it was or which was right. I do know that the Excel spreadsheet that I have to calculate my MELD score agrees exactly with my web calculator, but I wrote that too so I may have made the same mistake in both of them! The only other possible problem I noticed was that if I entered a number lower than 1 in yours (and in mine too actually), it dropped the MELD score down and if I remember correctly UNOS rounds any number below 1 up to one. So for instance my creatinine is .7, my bilirubin runs around 1.2, and my INR around 1.1. That gives a MELD score of 4.7 if I use the .7 for creatinine, but gives 8.2 if I use 1, which corresponds to what the transplant center calculates. You could probably just add a note to round numbers lower than 1 up to one, or you could make the program round them up automatically. I thought I'd fixed mine so it rounded up but I guess not! athan BTW, if you'd like to look at mine it's at: http://www.mindspring.com/~thegeorges/meld.html It has a couple old features (stuff about what type of liver disease etc.) because it was written before the MELD score was actually adopted by UNOS. I guess before, I just assumed everyone was using the UNOS one since it’s their MELD score that counts, but what if everyone is using a different one? Things that make me go Hmmmm…. I think the reason that you got different numbers was because of the rounding of numbers lower than 1 up to 1 like I mentioned in my previous email. I just checked 's updated calculation, and it now gives exactly the same answer as UNOS (and mine if do my rounding manually), so I think it's good to go! athan Quote Link to comment Share on other sites More sharing options...
Guest guest Posted January 17, 2005 Report Share Posted January 17, 2005 Barb, If you are listed, then UNOS's computer program automatically calculates the MELD score when the person at the center inputs it to send to data to UNOS electronically. Once the file is saved, it cannot be modified. (All of the rules having to do with how old labs can be, etc., require this kind of computer coding.) The reason I know this is because my MELD was recently input incorrectly by one of my centers (they typed my DIRECT bilirubin which was 6 instead of my total bilirubin which was 10). When they went to correct the error, they had to work directly with someone at UNOS to correct the entry. If someone isn't listed at a center, I'm not sure what method the center might choose to use to calculate MELD. I just know that for those of us who are listed, it automatically goes through the UNOS channels/calculations as part of keeping us listed.
