I think Raffa from FDNY is a member of this list and he might have the

actually script they use but FDNY's (and most agencies in NJ have used similar

scripts) goes something like this:

" The signal 5-5-5-5 has been transmitted and reads as follows....It is with

deep regret that the Fire Department of the City of New York annouce the death

of Firefighter ____________ of Engine (Ladder or Rescue, etc) Company ____

while operating at Box #_______ at (location) on (date). Funeral arrangements

are

as follows: __________________________. Signed __________________ Fire

Commissioner and _____________________ Chief of Department "

There have been slight variations of the script over the years. Here's one

from February 5, 1996:

IT IS WITH REGRET THAT ANNOUNCEMENT IS MADE OF THE DEATH OF FIRE FIGHTER

LOUIS VALENTINO OF RESCUE 2 AS A RESULT OF INJURIES SUSTAINED WHILE OPERATING AT

BROOKLYN THIRD ALARM BOX 2408 TRANSMITTED 1537 HOURS OF FEBRUARY 5, 1996.

SIGNED: HOWARD SAFIR, FIRE COMMISSIONER;

DONALD BURNS, CHIEF OF OPERATIONS.

Another variation:

IT IS WITH REGRET THAT THE DEPARTMENT ANNOUNCES THE DEATH OF (rank) (name) OF

(company) FROM INJURIES SUSTAINED WHILE OPERATING AT (borough) (alarm level,

if any) (box), TRANSMITTED (time), (date).

Funeral arrangements usually follow soon afterwards as a 65-2 message:

SIGNAL 65-2 IS TRANSMITTED, ALL UNITS STAND BY TO RECEIVE A DEPARTMENT

MESSAGE. (pause)

SIGNAL 65-2 IS TRANSMITTED, MESSAGE IS AS FOLLOWS: (text of message)

The script and the message are written by Operations. We do nothing but read

it out over the radio, voice alarm, and send it out on the teleprinters

citywide.
