It could also be misconstrued by the teed off as a form of work stress/harassment. I would run very far away from that idea. The evaluation could be good or bad<< I always found that a blank sheet of paper worked just fine...:-) All you need do is advise your staff that you are accepting feedback (good, bad or indifferent). All in an informal and " off the record " setting.< I'd give this some serious thought... You sure you want to do this? If a supervisor comes to me and discusses problems, I may not be pleased, but I'll know it's their job, and I'll accept it. If a co-worker drops an anonymous letter in a box about my work....
