UC/PSC - j pouch reconstruction

My spouse was diagnosed with UC AND PSC in 1989. In 2000, he had an

ilio anal pull through with J-pouch reconstruction. About nine

months after the second surgery, he was diagnosed with chronic

pouchitis and is on Levaquin. He just received a liver transplant

this past June. Things are going very well (although, I can relate

to Mike's wife). He's now discovering that his pouchitis is elevated

and not sure what the cause is. He's got a call into the transplant

coordinator.

My guess: He's not taking his meds as scheduled, not eating well,

keeping odd hours (sound familiar???). I was away from the house

this past week (International Barbershop Competition - ask me

privately about this!!) so I would also guess part of the problem was

I wasn't around to " remind " him to take care of himself. It should

be better this week.

I know if I skip just one day of antibiotics my pouchitis flares. My

guess is that either he was 'non-compliant' ie not taking the

levaquin daily or the new liver metabolism of levaquin is different

than before transplant so maybe he needs more? If he was on a 1/2

dose before now he might need a full dose. Also consider probiotics

(yogurt, FloraQ, FlorRestor, more) and see previous posts on

Xifaxan - a new non-absorbable antibiotic (works for me @ only once

a day).

A word of caution regarding compliance in someone who has a new

transplant. If you skip levaquin for pouchitis you get pouchitis. If

you skip transplant meds, you get rejection. I've seen patients lose

someone else's kidney when they decided to stop taking meds for a

couple of weeks due to marital issues :(

Anyway hope he's feeling better.

Has Lonnie tried any of the probiotics? So folks find help with these over the counter "bugs". I am in a similar boat having a Jpouch in 2000 after 27 years of ulcerative pan colitis.

Yogurt has been recommended by other doctors (PCP and chiropractor),

but he doesn't like the taste. I'll mention it to him again,

though. If he can have just one serving with a meal, it would help.
