Chad, I agree!  How could you even remember what you did with a client/patient

2 months ago? I have a PT manager friend that had an employee (DPT) that was

consistenly behind 1-2 months on notes.    He would then have to sit

for days to try and catch up.  It was unfair to the other staff who had to

see his patients for days while he tried to catch up on hundreds of notes. 

What if you have a Medicare audit and there are no PT notes in the chart?  Not

good practice.

Tell your client to fire that employee. What an absurd comment by that person.

Even if it were true and people practiced that " rule " they would be out of

compliance with many entities. Besides how can someone recall what he or she did

two months ago? I have everyone complete all notes the same day of treatment.

et. al -

And what about the deposition? The deposition. The legal accountability.

The one at which the opposing attorney asks you whether the documentation

was prepared " ... at, or reasonably proximal to, the actual treatment? "

One's recollections (and credibility) are best when they do the note during

or at the end of the treatment.
