Guest guest Posted June 3, 2010 Report Share Posted June 3, 2010 Chad, I agree! How could you even remember what you did with a client/patient 2 months ago? I have a PT manager friend that had an employee (DPT) that was consistenly behind 1-2 months on notes.  He would then have to sit for days to try and catch up. It was unfair to the other staff who had to see his patients for days while he tried to catch up on hundreds of notes. What if you have a Medicare audit and there are no PT notes in the chart? Not good practice.  Egbert PT Director of Rehab Draper, UT  Subject: Re: Documentation To: PTManager Date: Thursday, June 3, 2010, 6:35 PM  Tell your client to fire that employee. The one at which the opposing attorney asks you whether the documentation was prepared " ... at, or reasonably proximal to, the actual treatment?
