The road to recovery after an injury or surgery can be a long one. Physical

therapy can be a crucial tool in the healing process.

Physical therapy makes a big difference immediately following an injury or

operation. But once patients have less pain and are more functional, they

can still benefit from clinical exercise directed by doctors and trainers.

Regaining strength and mobility after an accident or intense surgery can be

a lengthy process. In many cases, patients might not have reached their

optimum potential after the few physical therapy sessions covered by

insurance.

" We are the bridge between the discharge from physical therapy and

I agree, this is outrageous.

However, we need to look at what our profession has done to allow this to

happen.

Many PT's focus on passive interventions (modalities, myofascial release, soft

tissue work..) for far too long in the clinic, rather than active interventions.

Personal trainers stress, market, and sell themselves as the ones who address

issues with active interventions (exercise, training), as well as educators on

how to exercise and use certain equipment.

Many physicians get frustrated with the excessive passive and redundant

interventions of physical therapy. Personal trainers market to physicians that

they will train and exercise their patient's while the physical therapist says,

" no, you are not ready to exercise because you have this trigger

point/dysfunctional joint.... that won't go away, we need to fix that first. "

We are cutting out our throat's, again. Personal trainers are not trained or

capable of dealing with pathology. Yet, they continue to move forward while we

PT's, in our starched lab coats, focus on a dysfunctional facet joint, or a

trigger point, without addressing the functional needs of the patient to

participate in life.

At what point will our profession treat patients as people in need of functional

restoration rather that perpetuating dysfunction.

PS, I have a clinic in a 140000 sq foot health and wellness center as well as a

free standing out patient ortho clinic and see patients 40 hours a week.

Why is the PT involved not doing the same thing. If your service is

seen as a benefit to the patient instead of a requirement by the MD, the

patient will not want to go somewhere else, especially someone who took

a weekend certification course to find out gym training, not how it

applies to different individuals depending on age, co morbidities, etc.

I do not let my patients do that until they are at the point where a gym

workout is good for them. At that point, I go with them and show them

what they can do. I communicate with trainers there, but they do not

continue my POC.

We have to sell ourselves. Call it an extended fitness plan or some

other program. There is nothing wrong with it, but the patient should

stay with the PT until goals have been met. You don't D/C PT just

because the insurance companies say so. If the patient will pay a gym,

they will pay a PT. Take advantage of the situation.

Hi everyone,

Beware of these programs as they are frequently run by physicians and as a

way of getting around the rules surrounding physical therapy. While it may

not be the case in this instance, many of these programs are designed as a

first choice, not as an aftercare program.

If I were in North Dakota, I would be asking the PT Board to look into the

legality of this as it should not be advertised as " medical " in any way.

Doing so encroaches on most PT practice acts or so it seems (I am not a

lawyer). The issue should be explored and with force to protect the

practice of physical therapy, but most importantly to protect the public as

there is no legal protection that PT licensure provides.

Would be an interesting case because I am sure that physicians would claim

to supervise these programs and that how are they different from a PT

supervising an aide or tech doing the same thing??

I also agree that all PT practices should be capturing this business by

providing the same service at their clinic by licensed individuals. Should

be a standard thing and because it is not at many clinics, that business is

being lost. And outpatient PT practice owners wonder why outpatient

practice is struggling??

Why is the PT involved not doing the same thing. If your service is

seen as a benefit to the patient instead of a requirement by the MD, the

patient will not want to go somewhere else, especially someone who took

Beware of these programs as they are frequently run by physicians and as a

way of getting around the rules surrounding physical therapy. While it may

not be the case in this instance, many of these programs are designed as a

first choice, not as an aftercare program.

Although you make a good point, the issues raised here are much greater than

what is called Physical Therapy and who is doing it.

There is a hugh liability issue for any professional (doctor, physical

therapist, whoever) taking responsibility for these activities.  By working

with them and issuing orders you are connected to them legally under your

license.  Why would any professional want to take that chance?

As an aside, if you are going to be liabile, then why don't you just continue

the treatment under private pay in your own facility.

The state board and the state associations should be all over this.

The point is they have no niche.  They want to provide a program under a

Physical Therapy guidance and under the guise of Physical Therapy.  This is not

a niche.  It is not a sound business plan.  It is ultimately a huge liability

issue for the therapist that might be sufficiently naive as to enter into

this.  If something negative were to happen (even a slip and fall) they fold

their tent and slip away into the night and the therapist is left holding the

bag.

Aren't there enough carpetbaggers around?

It's like some vitamin peddler claiming to be an extension of a physician and

prescribing for the doctor.  That is why the FDA requires the disclaimers.  If

not, then all sorts of people would engage in misleading information

intentionally designed to prey upon the uninformed customer.  This exactly

what physical therapists are not suppose to do.  This is also a great argument

for the AMA to let go of the strangle hold they have on PT by requiring referral

for treatment.

Wake up and smell the ointment my friend.  I'm sure that Ben-Gay would like to

claim it provides Physical Therapy by rubbing it onto the skin.

To the Group:

I have very interesting scenario...a PT clinic in my town was having a patient

do Ball exercises under a supervision of an experienced licensed PT. The patient

has done the prone extension exercises on the tx table without any problems and

does 3-4 reps of supine extension on the Ball. There was a lawsuit claiming the

patient " ruptured " a disc while doing the ball exercises which the MD

perpetuated in her notes that the " patient states that she had increase in back

pain after doing ball exercises " with a comment like.. " the exercises caused the

back pain " The settlement was slightly less than a $1 mil. Personal trainers do

not have the liability as we PTs do, so I can now imagine how difficult this PT

will have in the future prescribing any type of aggressive exercises. On the

flip side of the coin...passive modalities may not a bad idea as patients want

to feel good (that's where chiros come into the picture) I have noticed that

many a times patients themselves come with a plan of action and give a green

signal that they may be ready for aggressive exercises. As PTs we can definitely

observe our patients and make a decision that now they are ready for active

exercises.

I don't see anywhere that this person states they are performing the physical

therapy...they state they are the bridge between the two. I agree with another

persons posting....physical therapist can provide this exact service and should

prevent these patients going to their competitor....but as we always do is place

the blame on someone else that is trying to find their niche.

One other issue in regards to this thread: Does North Dakota have Direct

Access? This is an example of why full unrestricted Direct Access is

important because PT's can compete with personal trainers without the burden

of needed a referral for continued care once insurance is exhausted. If I

still practiced in a state without full Direct Access, I would be faced with

a large decision of whether setting up aftercare at my clinic on a cash

basis was worth the hassle of having to still get a physician referral to do

it.

Beware of these programs as they are frequently run by physicians and as a

way of getting around the rules surrounding physical therapy. While it may

not be the case in this instance, many of these programs are designed as a

first choice, not as an aftercare program.

If I were in North Dakota, I would be asking the PT Board to look into the

legality of this as it should not be advertised as " medical " in any way.

Doing so encroaches on most PT practice acts or so it seems (I am not a

lawyer). The issue should be explored and with force to protect the

practice of physical therapy, but most importantly to protect the public as

there is no legal protection that PT licensure provides.

Would be an interesting case because I am sure that physicians would claim

to supervise these programs and that how are they different from a PT

providing the same service at their clinic by licensed individuals. Should

be a standard thing and because it is not at many clinics, that business is

being lost. And outpatient PT practice owners wonder why outpatient

practice is struggling??

Why is the PT involved not doing the same thing. If your service is

seen as a benefit to the patient instead of a requirement by the MD, the

patient will not want to go somewhere else, especially someone who took

a weekend certification course to find out gym training, not how it

applies to different individuals depending on age, co morbidities, etc.

I do not let my patients do that until they are at the point where a gym

workout is good for them. At that point, I go with them and show them

what they can do. I communicate with trainers there, but they do not

continue my POC.

We have to sell ourselves. Call it an extended fitness plan or some

other program. There is nothing wrong with it, but the patient should

stay with the PT until goals have been met. You don't D/C PT just

because the insurance companies say so. If the patient will pay a gym,

they will pay a PT. Take advantage of the situation.

Hi everyone,

Beware of these programs as they are frequently run by physicians and as a

way of getting around the rules surrounding physical therapy. While it may

not be the case in this instance, many of these programs are designed as a

first choice, not as an aftercare program.

If I were in North Dakota, I would be asking the PT Board to look into the

legality of this as it should not be advertised as " medical " in any way.

Doing so encroaches on most PT practice acts or so it seems (I am not a

lawyer). The issue should be explored and with force to protect the

practice of physical therapy, but most importantly to protect the public as

there is no legal protection that PT licensure provides.

Would be an interesting case because I am sure that physicians would claim

to supervise these programs and that how are they different from a PT

supervising an aide or tech doing the same thing??

I also agree that all PT practices should be capturing this business by

providing the same service at their clinic by licensed individuals. Should

be a standard thing and because it is not at many clinics, that business is

being lost. And outpatient PT practice owners wonder why outpatient

practice is struggling??

Why is the PT involved not doing the same thing. If your service is

seen as a benefit to the patient instead of a requirement by the MD, the

patient will not want to go somewhere else, especially someone who took

a weekend certification course to find out gym training, not how it

applies to different individuals depending on age, co morbidities, etc.

I do not let my patients do that until they are at the point where a gym

workout is good for them. At that point, I go with them and show them

what they can do. I communicate with trainers there, but they do not

continue my POC.

We have to sell ourselves. Call it an extended fitness plan or some

other program. There is nothing wrong with it, but the patient should

stay with the PT until goals have been met. You don't D/C PT just

because the insurance companies say so. If the patient will pay a gym,

they will pay a PT. Take advantage of the situation.

The road to recovery after an injury or surgery can be a long one.

Physical

therapy can be a crucial tool in the healing process.

Physical therapy makes a big difference immediately following an injury

operation. But once patients have less pain and are more functional,

I went to the company's Website but could only ascertain that the

" Physical

Therapy " intervention was performed by personal trainers. Is that

acceptable in North Dakota?
