We are a CARF accredited facility and use the Lifeware outcomes management tool.

(A division of UDS). We have developed a custom SCI form which is nationally

benchmarked. It does address all of the parameters you are referring to.

share their Outcome Measurement Tool as required by CARF?

Are there any SCI outcome measurement tools related to balance, mobility, and
