Utilization review - reposting

Reposting my original message below because it seemed to get taken to another

unrelated thread and I never got any response. Still need help with this if

anyone has advice:

I would like to formalize the utilization review/chart audit process at my

hospital-based outpatient clinic. Can anyone describe in detail the process they

use, any forms they use and what they do with the information after the audit is

done? I'd specifically like to know what action is taken if it is determined

after chart audit that a therapist needs to make corrective changes.
