Fall prevention/balance PT -- referral process

Does anyone have experience with marketing outpatient PT for treatment of

history of falls?

My billing office tells me that the ICD9 code for history of falls is

insufficient to justify PT, and that there needs to be a primary dx.

We would like to do outreach to PCP offices to let them know about PT treatment

for balance impairments/fall history. How should they write their referrals in

order to ensure payment for this care in an outpatient setting?
