To balance out Jeff's view, I can say I have seen 15+ years of satisfied and

productive therapists in the outpatient settings I have worked in. Most

clinicians work 4 times 10 hours, love the extra day off, cherish having one

less commute and quickly seem to adapt to the longer days. The longer days offer

more options to our patients and more efficient use of our clinical space.

I would run a less successful operation and would have less satisfied employees

if I would no longer give them the 10 hour day option.

positive. I own a small clinic, where we do one on one sessions, 45 min on

average. I have one PT who works 4 long days, and I myself have often pulled 10

+ hours in the clinic. My observation is that 10 hours is a long time to work in

this sort of setting. When you do a lot of manual work, spending indepth time

with each patient, you are pretty tired at the end of 8 hours. I have felt as

though I had to struggle to provide the same caring quality in the 9th and 10th

hour. My observation of staff is that 10 hours x 4 does not produce as well as

8 hours x 5, unless you personally and aggressively manage productivity. The

tired PT, by the 4th day, does not care about trying to fill that spot that

opens up with a cancellation. They are just happy to have a break.

I own an outpatient practice and we currently have our therapists work 5, 8 hour

days. We are looking at changing to 4, 10 hour days. I am trying to figure out

what the pros and cons are for each of these options. We do see patients for

one hour each.

What issues or problems have you seen because of either decision you have made?

What positive things have you seen because of your decision?
