Word help for ADHD patient

The patient has ADHD. The doctor says the patient is to return with

**s/l actors** questionnaires completed by two teachers.

Any suggestions?

I bet that's it. I found hits on Google for it in relation to ADHD and it being

a score on task and academic efficiency.
