Wording of physical exam

How would you word the following physical exam? I don't have my reference

materials with me (not at my usual computer/office), and I feel all alone...LOL.

Would you make each of these sentences????

Physical exam: The patient is a pleasant well-developed, well-nourished male in

some acute distress. Head atraumatic, normocephalic, pupils are round and

reactive to light, extraocular movements full, lungs clear to auscultation

bilaterally, cardiac regular rate and rhythm without murmurs. Examination of

the lower extremities normal muscle tone and bulk, normal motor strength 5/5 of

the bilateral lower extremities, sensory exam (s/l) intact to light touch and

pinprick, deep tendon reflexes are symmetric 2+/3.

Yes, I would make them separate sentences. I would probably add some

colons, too, to make subheadings within the paragraph. JMHO.

Physical exam: The patient is a pleasant well-developed, well-nourished male

in some acute distress. Head: Atraumatic, normocephalic. Pupils are round

and reactive to light, extraocular movements full. Lungs: Clear to

auscultation bilaterally. Cardiac: Regular rate and rhythm without murmurs.

Examination of the lower extremities: Normal muscle tone and bulk, normal

motor strength, 5/5 of the bilateral lower extremities. Sensory exam:

Intact to light touch and pinprick. Deep tendon reflexes are symmetric,

2+/3.
