EMS providers may now opt to use this affidavit to submit their Calendar Year 2002 emergency healthcare runs to BEM for FY 04 Designated Trauma Facility and Emergency Medical Services (DTF/EMS) Account funding purposes only. If a provider does not submit an affidavit by June 30, 2004, BEM will utilize the provider's run data information available from the State EMS/Trauma registry. Note: EMS providers are still required to submit run data to the State EMS/Trauma Registry. This affidavit is for funding purposes only.
