All,

This is just a reminder that the due date to submit the FY04 DTF/EMS-EMS

allocation affidavit mentioned in the email below is June 30, 2004.

to see a copy of the cover letter and affidavit we mailed on June 1, 2004 to

all eligible EMS providers.

EMS providers may now opt to use this affidavit to submit their Calendar

Year 2002 emergency healthcare runs to BEM for FY 04 Designated Trauma

Facility and Emergency Medical Services (DTF/EMS) Account funding purposes

only. If a provider does not submit an affidavit by June 30, 2004, BEM will

utilize the provider's run data information available from the State

EMS/Trauma registry.

Note: EMS providers are still required to submit run data to the State

EMS/Trauma Registry. This affidavit is for funding purposes only.

It was brought to my attention that most people were unable to view all the

verbiage contained in email I sent earlier regarding the Affidavit for the

FY04 DTF/EMS-EMS allocation. Due to this, I am resending the email. Sorry

for any confusion.
