Documenting discharges due to CAP

How are other physical therapy clinics documenting patient discharges due to the

patient meeting their Medicare CAP and not wanting to pay privately, or continue

at a hospital based outpatient facility? I have previously worked at facilities

that strongly encouraged NOT including wording related to financial issues in

the discharge summaries.
