Medicare question about PT evaluations

Hi,

Been searching for Medicare policy information regarding PT evaluation but

having trouble finding the answer to this;

Patient seen at one clinic for X treatment diagnosis and a full evaluation

and certification is done. The patient then stops treatment for a while.

Same patient goes to a different clinic for same diagnosis after the a few

month break from therapy. Does Medicare limit the billing of a second

evaluation for the same diagnosis?
