The Plan of Care in the SNF must be reviewed and approved by the referring

physician timely. There is no individual certification requirements for Part

A SNF therapy services as the facility is required to obtain certification

for the skilled level of care which includes all services provided,

therefore the services for rehab are already certified by default.

Therefore, unless required by your State Practice Act, you make the POC/POT

for the length of time which you anticipate it will take, exactly like the

instructions for Part B, with the exception that it can be longer than 90

days.

The 30 day recert of Part A services began in the 90's when facilities

started using the 700 form (which was an Outpatient, not Inpatient form) and

followed its regulations. So to summarize,

1) There are NO Medicare regulations for individual certification and

re-certification of therapy treatment in SNF Part A

2) The plan must be reviewed and approved by the physician

3) The plan is in effect for the period identified as the duration on

the plan and does not require updating by Medicare

I would have to check the Medicare regs to see what is the requirement. I

can only say that we have been recerting long-term SNF Pts for part B every

30 days for several years.

Can you please clarify this information.

Does this same rule apply in a free standing clinic?   

Does a patient still have to see there doctor within 60 days of the

original order?
