Guest guest Posted June 7, 2010 Report Share Posted June 7, 2010 last night I had a bedtime glr of 90, so I did not dose the prescribed 30 units of Lantus. This morning my fasting glr was 164. Since the prescribed dose of Humalog is: fasting glr over 150 subtract 100 and divide by 50 which rounded off to the nearest whole number is 1. So I add the 1 to the prescribed dose of 15 to get 16 units dosage of Humalog for the pre-breakfast of 60 grams of carbs. I follow the same formula for the pre-lunch of 60 grams of carbs. formula pre-lunch glr 201 lunch 60 carbs Humalog dose 17 units afternoon snack beer 13 carbs pre-supper glr 216 Humalog dose 20 plus 2 equals 22 bed time glr 222 new Lantus dose35 units Quote Link to comment Share on other sites More sharing options...
