Fluconazole/PANDAS question

My 15 yr old PANDAS kid is having his biggest backslide in 3 years. Fluconazole

100mg BID has always taken care of the anxiety within a day or so, afterwhich I

slightly increase his OLE to knock out the OCD (he's a viral kid). This protocol

isn't working this time. Is it possible that the Fluconazole dose isn't high

enough? (He's been on it for 20 days with 20 scheduled yet to go) Do we need to

rotate to Nystatin? Fluconazole could, historically, be considered a miracle

drug in this house, I'm really confused as to why it's not working this time for

the same old symptom cluster. Many thanks for any advice.
