She used similar examples to the ones you provide. Guest guest Posted July 27, 2009 Report Share Posted July 27, 2009 The OASIS form itself defines skin lesions as " pathologically altered tissue " so unless one considers freckles and moles to be the result of a disease process, they ought to be left off the form. More important than the definition, however, is the true clinical relevance of the form's content. Kicking off a therapy session by completing a 17-page form (soon to increase in size and scope if CMS has its way, and it always does) is an abominable way to provide medical care. It is incumbent upon all of us that we, at very least, do not allow ourselves to be carried away on this rushing tide of bureaucracy and begin over-complying. Doing so helps neither patients nor providers.
