The following is request from a research student working under Kim Blake in

Halifax NS. They are looking for research participants and asked for my

CHARGE- Osteoporosis study- Dr. Kim Blake

Dear Weir,

My name is Forward and I am a research student working under the

direction of Dr. Kim Blake at the IWK Health Centre. We are currently

conducting a study examining Osteoporosis in Adolescents and Adults with

CHARGE syndrome. The questionnaire for this study has been developed over

the

past year and has ethical approval by the hospital and Meg Heffner of CHARGE

USA.

Unfortunately, we have been having a difficult time locating individuals

with

CHARGE syndrome (13 years and older) to take part in the study. Dr. Blake

suggested that perhaps you might know of individuals who may be interested

in

participating. Although, you are not obliged to assist us, your input would

be greatly appreciated.

Attached to this email is an information form will providing you with some

additional information as well as the questionnaire. If anyone that you

know

may be interested in participating, we would be appreciate it if you would

forward them the information or alternately, we would be pleased to send

them

a copy of the questionnaire either by email or by regular mail.
