Does anyone have a good one that works for them that I can use as a guide line? I appreciate any help that I can get with this project. You can fax or email me with the information below. Sometimes it is better than at others, and improving it without creating divas on *either* side of the table will be best for the practice. That said, an annual evaluation should be focused on the tasks and performance which were specified last year (or upon employment) in the job description. An employee should always know what they're going to be judged on. They should also know what the standards are for each component of the evaluation. Merry Christmas!
