Therapy Week for Medicare Part A SNF

I am looking for a reference that indicates the therapy week for patients

receiving part A services under Medicare. The RUG levels with minutes and days

per week are very clear. The therapy week is elusive. Is this the day of

admission through day 7, Sunday through Saturday, or some other variation?

CMS in one of their open door forums gave the definition of a Medicare week

as 7 consecutive days starting with the day of admission. We have taken that

one step further to identify that the week for the therapy POT is 7

consecutive days starting with the day of evaluation. So if you see the

patient on a Tuesday and write your plan for 5 days a week, then you can see

the patient Tuesday, Wednesday, Thursday and Friday, miss Saturday and

Sunday and then start again on Monday to complete your established

frequency. If you continue to see the patient on Saturday, then you would

have to miss Monday to be compliant. After that first week, you will just

migrate to the Monday thru Friday treatment.
